We work hard to keep your information safe, using secure systems and checks.
Your Choice
You don’t have to take part. At the start of each form we provide some information about how your information will be used by the healthcare team, and we ask:

- If you say Yes, your answers are saved and shared with your healthcare team.
- If you say No, nothing is saved.
