In healthcare, we use the word transition to describe the process of preparing, planning and moving from children’s to adult services.

We understand that for your patients, moving away from a team of doctors and nurses that they have been with for many years can be scary but hopefully, by getting involved in the transition process, you can empower them to feel more confident and happier about the move.

To support clinicians to deliver this this transition, we developed the Ready Steady Go transition programme.

Who is it for?

Any child over the age of 11 with a long term health condition

Why?

Shown to improve long-term outcomes
Helps your patients to take ownership of their health needs and be fully informed before their transition to adult services

How?
